Your Doctor at Warrina Medical Centre in conjunction with yourself will plan and coordinate your Health Care needs.

These needs may include multidisciplinary team-based care - Specialist care, podiatrist, dietitian, diabetic educator, occupational therapy, physiotherapist and other allied health professional services. 

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cardiovascular disease, diabetes, musculoskeletal conditions, endocrine and metabolic disorders.

Our Practice Nurses assist with providing ongoing support and follow up. These are performed through examinations / interventions as indicated by your Health needs.
